観音は種々の変化身を現して衆生を救うと信じられた。十一面観音はその変化観音の一つである。十一面観音神呪を熱心に受持・読誦することによって、除病・滅罪の功徳が得られるという、比較的理解が易しい経説であったために、時代を通じて広く浸透し、彫像や仏画に多くつくられてきた。本図は、頭部に十一面の仏面を表し、左第一手には開敷蓮華を持ち、左第二手は水瓶を、右第一手は念珠を握り、右第二手は施無畏印とする四臂につくり、須弥山の上の蓮華座に結跏趺坐する坐像に描かれる。さらに、上部左右に大日如来坐像（胎蔵界と金剛界）の二尊、下方左右に、白牛に乗る大自在天と踏割蓮華座に片足を乗せる軍荼利明王を配する。こうした五尊形式の十一面観音像は遺例少なく、室町時代・15世紀の作例ながら、貴重な存在といえる。
