不動明王を中心として、下方左右に降三世（右下）と軍荼利（左下）、上方左右に金剛夜叉（右上）と大威徳（左上）の四明王を１幅にまとめて描く五大明王像である。これは、古来、毎年正月８日から14日までの７日間、教王護国寺の長老が阿闍梨として参内して宮中の真言院にて鎮護国家・玉体安穏・五穀豊穣などを祈る密教の加持祈祷の修法の本尊として用いられてきた。古い遺例は、五尊をそれぞれ単独の尊像につくられた（教王護国寺・醍醐寺蔵）。が、これは、１幅にまとめる。五尊いずれも火焔光背を背にして、中尊の不動明王は、岩座を意匠化した瑟々座に結跏趺坐する。降三世は大自在天夫妻を踏み敷き、軍荼利・金剛夜叉は片足を蹴り上げる姿勢に、さらに、大威徳は６足をもって水牛に乗る形に描かれる。色彩や図様の形式化などから、室町時代・15世紀の制作と推定される。なお、この１軸は、真言宗智山派の酒波寺（滋賀県高島市今津町）の什宝であったと伝えられる。
