観音は種々の変化身を現して衆生を救うと信じられた。十一面観音はその変化観音の一つである。十一面観音神呪を熱心に受持・読誦することによって、除病・滅罪の功徳が得られるという、比較的理解が易しい経説であったために、時代を通じて広く浸透し、彫像や仏画に多くつくられてきた。頭上に、『十一面神呪心経』に基づいて、菩薩面・瞋怒面（眉を吊り上げ、口をへの字に結んだ怒りの表情）・狗牙上出面（結んだ唇の間から牙を出す姿）を描く。左第一手には蓮華を活けた花瓶を、左第二手は水瓶を、右第一手は念珠を握り、右第二手は施無畏印とする四臂の立像。格狭間をもつ礼盤の上に置かれる蓮華座に立つ。また、光背には、蓮華座に乗る宝珠を11個、金泥で描いている。さらに、下方左右に、不動明王と毘沙門天像を従える三尊形式をとる。彫像ではあるが、牛伏寺（長野県）や志度寺（香川県）、伊勢廻寺（滋賀県）の本尊十一面観音像が両尊を脇侍に従える形式と同じ趣旨によるものと考えられる。室町時代末期・16世紀の制作と思われるが、図様の珍しい遺例として貴重な存在といえる。
